To set up an initial consultation, you can call me at (310) 995-2628. The address is 2200 Pacific Coast Hwy, Suite 204, Hermosa Beach, CA 90254. The office is one block south of Artesia Blvd, directly after Sepulveda Blvd becomes Pacific Coast Hwy.

INFORMATION FOR NEW CLIENTS

For clients with PPO insurance plans, I will bill and send in the paper work to your insurance company. This will help you get reimbursed directly.

Please arrive 10-15 minutes prior to your initial appointment to read through and sign the appropriate forms. They will be left for you in the waiting room. If you are a couple, each person should fill out and sign the appropriate areas of the forms. Once you have completed the forms, please turn the light switch up next to the interior door.

Free visitor parking is available in the front of the building in the Visitor Parking Lot. You can also check if there are open spots in the back of the building on Borden St. by making a Left directly past 2200 PCH onto 21st and then another Left onto Borden St. Please read parking signs for parking availability.
